IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Form 1095‑C provides information about the health insurance coverage offered to full‑time employees and is required under the Affordable Care Act. Under the Paperwork Reduction Act, employers are permitted to make Form 1095‑C available upon request instead of automatically mailing it. If you would like to receive a copy of your Form 1095‑C, you may request one at any time. Your form will be provided within 30 days of your request, as required by federal law. Please contact the Benefits Department to request your form or to ask questions about your coverage information.
